My office is located in the historic Borough of Malvern less than a third of a mile walking distance from the SEPTA R5 train and SEPTA 92 bus stop.  The office is also easily accessible from the EZ Pass Entrance for Hwy 276/76 and Hwy 202 Exits both off of Morehill Road/Hwy 29. Parking is free.

  • Please review all of the forms above and print, fill out and sign the applicable documents prior to your first session.

  • Preprinting and filling out your forms before your first session will help expedite your first session allowing us more time to work on your presenting issues.

  • Treatment cannot begin until all required forms are completed and signed. Please note: If you are divorced and both parents require signature, please make sure both parents sign the appropriate forms and provide copy of divorce decree. If you are an appointed legal guardian for the individual, please provide guardianship legal forms.

What are insurance, rates & payment?

Insurance  

Please check with your insurance provider to verify your coverage before your first session. I am an out-of-network provider.  Some insurance companies may reimburse up to 98% or more of this fee. You will be provided with a receipt with all necessary information to file your claim with your provider.   Therapy may be covered with your HSA (Health Saving Account).

Hourly Rate  

  • Your investment is $150 for a 55 minute session. Please note: Some insurance companies may reimburse up to 98% or more of this fee. Please confirm with your insurance company first.

Payment

  • Payment is required at the time services are rendered and can be made by cash, credit card, or by check to CREATE in Greater Philadelphia, Inc.

  • I accept credit cards such as Visa.

  • Insurance does not cover missed appointments.

  • A 48-hour advance notice is required for cancellation on all appointments or a full hourly fee will apply as I cannot fill an appointment time once it has been reserved.

  • I will provide you with an invoice called a "super bill" with all necessary information needed for filing your "out-of-network" claim.

  • Most health savings plans may be applied for mental health services.

Are my sessions confidential?

Confidentiality & Privacy Policy

The law protects the relationship between a client and a psychotherapist.  Information cannot be disclosed without written permission.

      Exceptions include: 

  • Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.

  • If a client is threatening serious bodily harm to another person(s).

  • If a client intends to harm himself or herself, I will make effort to enlist their cooperation to ensure their safety. If they do not cooperate, I will take further measures without their permission provided to me by law to ensure their safety.
